Nurse's Office
A registered nurse is available at both campuses to assist with student health concerns. The school nurse supports the health and safety of all students, particularly those with chronic or severe medical conditions. All students who sustain an injury during class or on school grounds must report immediately to the nurse for evaluation and care. If a student feels ill and wishes to leave school, they must first see the registered nurse to be assessed before leaving. This ensures proper care and safety protocols are followed. At the South Campus, if a student cannot safely use the stairs due to an injury or health condition, they may obtain an elevator pass from the nurse. However, an elevator pass is only granted with a medical accommodation note from a physician. The South Campus nurse may issue a one-time daily elevator pass for temporary situations. A physician’s note will be required for continued accommodation if additional days are needed.
Certificate of Child Health Examination
In accordance with the Illinois School Code, all students entering as freshmen or transferring into an Illinois school must complete a physical examination performed by a licensed physician (M.D. or D.O.), an Advanced Practice Nurse (APN), or a Physician Assistant (PA). This physical exam must be documented on the Certificate of Child Health Examination form, which is provided by the Illinois Department of Public Health.
Key Requirements:
Physical Exam: The completed physical exam form must be submitted by the first day of school or uploaded into Powerschool during the registration process. Complete Form: The Certificate of Child Health Examination must be fully completed, including the Parent/Guardian section. This ensures all necessary information has been provided.* A sports physical will not be acceptable as a 9th grade physical.
Immunizations: Documentation of immunity to or vaccination for the following diseases is required: Tdap (Tetanus booster), Dtap (Diphtheria, Pertussis-whooping cough, Tetanus), Meningitis, Polio, MMR (Measles, Mumps, Rubella), Hepatitis B, and Varicella (Chicken Pox). Note: If a student has a history of varicella disease (chickenpox), verification from a healthcare provider can be submitted in lieu of the vaccination.
Seniors: Require a 2nd Meningitis vaccine prior to the start of their senior year for compliance.
Compliance and Exclusion: Per Illinois School Code, no student will be allowed to attend school unless they have submitted the required physical examination documentation and proof of all necessary immunizations. Failure to comply with these requirements by October 15th will result in the student being excluded from school attendance until all required documentation is provided. Please ensure that all forms are completed, signed, and submitted by the required deadline to avoid any disruption to your child’s education. The health and safety of our students are a priority, and we appreciate your cooperation in meeting these state requirements.
Vision and Hearing Screening Requirements
By Illinois School Codes, vision and hearing screenings will be conducted for all freshmen during the semester in which they participate in physical education, as well as for any new students enrolling in the district. It is important to note that the vision screening conducted by the school is not a substitute for a comprehensive eye and vision examination by a licensed optometrist or ophthalmologist. A student will be exempt from the required vision screening if a report form, completed and signed by a licensed optometrist or ophthalmologist, is submitted to the school. This report must indicate that a complete eye examination was performed within the past 12 months and that the evaluation results are on file at the school.
Emergency Action Plans and Self-Administration of Medication
In compliance with the Illinois School Code, the School District permits students with asthma to self-administer medication, provided that a completed authorization form is submitted by the student's parent or guardian. For students with medical conditions such as Diabetes, Seizures, or Severe Allergies (requiring the use of an EpiPen or other emergency medication), it is critical that an Emergency Action Plan be completed and submitted to the school. This plan is necessary to ensure the immediate and appropriate care of your child in the event of an emergency. Having a current Emergency Action Plan on file helps school staff respond quickly and effectively to medical emergencies, ensuring the safety and well-being of your child while at school.
If your student needs prescription medication during school hours, please complete the Prescription Medication Authorization Form, signed by both the physician and parent/guardian. In accordance with the Illinois School Code, students are permitted to self-administer non-prescription medication. To authorize this, please complete the "Over-the-Counter Medication Authorization" form in PowerSchool. The link is under the "General" tab in the Student Forms section.
Concussion Information
School nurses and athletic trainers play critical roles in managing student concussions. Clear communication between the school nurse, athletic trainer, parents, and healthcare providers is essential for effective concussion management.
